Most expressways (高速道路, kōsoku-dōro) are toll-operated by NEXCO East, Central, West and regional operators. The standard rate for a passenger car is roughly ¥25 per km, so a Tokyo–Osaka run costs well over ¥10,000 one way. Toll booths accept cash or ETC (electronic toll collection) — cash is slower, and only ETC gets the night and weekend toll discounts.
At rental time, select 'ETC card' (¥330/day) as an add-on. The operator issues a credit-linked ETC card that works with the in-car ETC reader. At each toll gate, drive slowly through the purple ETC lane — gate lifts automatically, toll is added to your rental invoice. No need to stop, slow to 20 km/h.
Flat-rate expressway passes for foreign visitors driving a rental car: the Hokkaido Expressway Pass (HEP), the Central Nippon Expressway Pass (CEP) and the Kyushu Expressway Pass (KEP), plus the nationwide Japan Expressway Pass. Each is sold for a set number of days and works only with an ETC card. Prices change from year to year, so check the current fare on the operator's official page and compare it with the tolls on your route before you add one.
On a short same-day trip with only a few hundred yen of tolls, the ¥330/day ETC add-on may not pay for itself. Cash lanes are marked 一般 (ippan, 'general') in green. Be aware that city networks such as Tokyo's Shuto Expressway are converting more and more gates to ETC-only, so if your route enters a big city, rent the ETC card.
Every 30-50 km on major expressways. Service Area (SA) = toilets, convenience store, restaurant, petrol, often onsen and local produce. Parking Area (PA) = toilets + vending. Famous SAs: Ebina (Tōmei), Adatara (Tōhoku), Kariya Highway Oasis. Many service areas also offer free Wi-Fi.
