Japanese law requires children under 6 to use a suitable child seat. The fee is 540–1,000 JPY/day depending on the type; reserve one when booking the car.

Rear-facing seat with head support. ~880 JPY/day. Request it when booking — not all cars have one available.
Forward-facing seat with a 5-point harness. 540 JPY/day. The most common type — available from most rental companies.
A booster cushion that positions the car’s seat belt correctly across the child. ~440 JPY/day. Children aged 6+ are not required to use one, but it is still recommended up to age 10 / 140 cm.
